All students participating in school-sponsored sports (grades 5-12) will need to have a physical on file with the school. The physical must be completed annually, with new emergency treatment and interscholastic sports exam forms placed on file with FACS each year.
The physical may be completed with your family physician. The Little Clinic at Kroger also offers athletic physicals at a cost of $29.
IMPORTANT FORMS:
Prior to getting a physical, please download and complete the physical questionnaire below. Take the questionnaire and the interscholastic sports exam form (also below) with you to the doctor's office or clinic. If you are going to the Little Clinic at Kroger, please download the Little clinic coupon and take it with you as well. Finally, you will need to turn two forms in to the school office: the interscholastic sports exam form (signed by the physician completing the physical) and the emergency treatment consent form (completed and signed by parent/guardian).
